Design
General views of the design are required, if possible front, top, side, back and bottom views. For Arab and Turkish citizens, a simply signed power of attorney is required. For citizens of other countries, a notarized power of attorney must be submitted. Multiple design applications can be filed. A description of the design in Ukrainian must be submitted.
